People Machine Interface - Running Over Scrap Material

Incident Date: 07/16/2020 

Incident Description:  Operator ran over long piece of rebar. rebar got stuck in material handler's tracks. operator was unaware of this and continued to travel the machine. rebar came around with the machine tracks and whipped up and into the back window shattering the glass. operator was not physically injured.

Corrective action:  Improvements in housekeeping of equipment pathways. Operator was coached on jobsite awareness and the effects of running over scrap material.
