Transportation Incident - Fork Truck Operation

Incident Description:  Fork truck operated attempted to remove a box of cans from a storage rack.  Did not get the forks far enough into the pallet to pick up the load securely. The load caught on the storage rack narrowly missing from upsetting the rack.

Corrective Action:  Secure the rack to the floor to avoid upsetting the entire rack. Instruct all to stop and contact supervisor when load becomes unstable.  Lack of PPE could have resulted in injury.
