Incident Date: 05/30/2019
Incident Description: A shopping cart was brought in as scrap. It was possibility a newer type that had a feature built into the wheels that had batteries in it of some kind. These batteries caused a small fire that posed no risk but could have had major consequences had that cart made it into the scrap pile. See pictures below.
Root cause: Batteries in grocery cart wheels
Corrective Action: Educate employees and create awareness of this new type of scrap.
